Source / creator: BCBS Minnesota, HealthPartners and UCare Transparency in Coverage in-network files (45 CFR 147.212), October 2026; NPPES for clinic identity and location.
Collection method: Payers publish rates per provider NPI; clinics negotiate as practices. We rolled every NPI up to its clinic (org NPI sharing the TIN, then same practice address, then the TIN group) before taking anything, so a three-therapist clinic is one row, not three. Each clinic's rate per payer per code is the median of the distinct base (no-modifier, professional) negotiated rates seen across its NPIs; for BCBS Minnesota it is the Aware (broad PPO) network. The market table gives the per-code distribution across clinics for each payer.

About this data

The clinic-level table behind PT Rates: every Twin Cities physical therapy clinic we could match in the BCBS Minnesota, HealthPartners and UCare Transparency in Coverage files, with the rate each payer pays that clinic for each PT and E/M code, side by side. One CSV, 12,682 rows; column meanings in the schema. Clinics are named; individual therapists are not (no individual NPIs or names). Clinics that bill only under a therapist's own NPI appear as 'Group of N therapists'.
